School FluMist/Flu Shot Clinic Registration
Please read all information carefully
- Registration closes on Wednesday, September 27th. Don’t delay, register your child today!
- You may select any open appointment time slot but note that appointment times are not the actual time your child will be vaccinated.
- NEW: The Health Department is able to accept United health insurance this year.
- If you select “I don’t have insurance” a fee will be assessed: $20 per person. The Health Department will follow-up with you. A fee will not be collected due to financial hardship.
- To receive clinic reminders/communication, please fill in your email and cell number.
- Need to cancel? Use the cancel button in the email/text you receive after registering your child.
- If after registering you need to change the type of flu vaccine your child will receive, cancel the appointment, and call the Health Department at 781-721-7121.
- If your child is absent the day of the school flu clinic, please notify the School Nurse.
Click on your child’s school below to register them for the flu clinic to be held at their school. If you have children in multiple schools, please register each child separately using their school’s link.
